ENZSA Forms

These forms have been posted here to provide "one-stop-shopping" for coaches, players and soccer directors.  While some of them are aimed specifically at Xtreme players and coaches, please feel free to download them and modify them to suit your needs.

Where submission to the Zone Office is requested, either fax or hand-delivery is suggested.

If you have any useful forms geared towards Coaches and Managers, please send an electronic copy to the webmaster, and we'll include it. 

Forms for Soccer Directors

Form Notes
Soccer Director Information Sheet Please complete a new information sheet when you are elected.  Submit to the Zone Office.
NEST Volunteer Sign-Up Sheet Please print and complete at your registrations.  Submit to the Zone Office with your team declarations, or seperately.
ASA Registration Form Complete electronically, print and sign.  Submit the ORIGINAL to the Zone Office.  A copy signed by the Zone and EMSA registrars will be returned to you when the process is complete.
ESA Facility Waiver This form must be completed and signed by the parents of all minor players for each indoor season.  When completed, submit it to the Zone Office.

Forms for Coaches & Managers

Form Notes
EMSA Transfer Must be completed and submitted for any player on your roster who resides outside of the North Zone.  Print, complete, sign and submit the original to the Zone Office.  A copy signed by the Zone and EMSA registrars will be returned to you when the process is complete.
EMSA Trialist Release This form must be completed for each game for guest players on your roster.  A team official (Coach, Assistant Coach or Manager) listed on the releasing team's ASA Registration Form must sign it.  You must bring it with you to your game.  A signed letter of release can stand in place of this form.
Player Goals and Attributes This is a very useful tool for getting to know your players.  It is geared towards players U-10 and up.
Codes of Conduct A definite must - define your expectations ahead of time, and you're much more likely to have compliance.
Practice Plan Template This is a good place to start when planning your practices.  As players get older or more competitive, a section on conditioning should be added.
Player Skill Charts This is a very comprehensive chart for tracking your players' skill development progress.  We recommend checking out what most kids are able to accomplish by Age or Skill by going to The Coaching Corner skill charts page.
Outdoor Field Diagram Definitely a nice-to-have when preparing line-ups.

Forms for the Xtreme Program

Form Notes
Xtreme Tryout Registration Completing this form and bringing it to your first tryout makes the registration process faster.
Player Coach Evaluation* We encourage players to complete this form and submit it to the Zone Office, or to your team manager.  It helps us select coaches for later seasons, and is a useful development tool for coaches.
Parent Coach Evaluation* We encourage parents to complete this form and submit it to the Zone Office, or to your team manager.
